Contact is a charity that provides information, advice and support to families across the UK. They support families with disabled children and children with additional support needs, or who have health and rare conditions, from 0 to 25 years old. Families do not need a diagnosis or an assessment for their child to get support from Contact.
Contact also provides opportunities for parents to volunteer and advocate to ensure their voices, concerns, and challenges are heard by politicians and decision-makers.
They are carrying out this survey to find out more about the information and support needs and parent voices in decision-making and policy, so they can improve what they do as a charity for families with disabled children and children with additional needs.
This survey should only take five to ten minutes to complete, and almost all the questions are tick-box
